This evaluation is intended for the university to obtain feedback from students in order to continue offering quality teaching/services. Please take a few minutes to fill in this form as objectively as you can. All responses will be treated confidentially.

NOTE: Before you fill the form kindly read the following instructions;

1. Fill this form for all Courses/Units you have learnt this semester (i.e. if you have done 7 courses in a semester, your fill the form 7 times).
2. Once you fill and submit the form for the first course/unit, you are supposed to fill another form for the second course until you are through with all the courses done in that semester.
3. In 2. above, when you submit the form for the first course you will be asked to edit OR submit another form. For this case you are supposed to submit another form when entering the evaluation for a different course/unit and do the same for all of your courses in the semester.
